In the event that you have completed a prearrangement form and paid for your services through Tender Heart Pet Memorial please inform your Veternarian of the prearrangment, and let them know you have paid for your pets cremation. You will need to call us as soon as possible so we can arrange a pick up. *Please Note: A thirty day notice is recommended for prearrangement. In the event that your pet passes prior to the recommended time and your form and payment has not been received your balance is due when your pet is pick up for the cremation.
You may register your pet with us by completing the prearrangement form and mailing it to the Tender Heart Pet Memorial 210 Two Oaks Drive, Nicholasville, Ky 40356, or just stop by our facility to complete the necessary form. The price of your prearrangement will be honored no matter how long it is before you need our services.
